Krista Vernoff [the showrunner] thought of Meredith having a dream sequence at the beach, and asked if there was some dead character she could meet in there. And Ellen Pompeo pitched Derek.
Ellen personally asked Patrick if he would come back on the show. She said that through this dark times and the premiere also being COVID heavy, they wanted to do something for the fans of the show.
