Assemble
1) None of the established heroes have secret identities. Either their dual identity is known, they're S.H.I.E.L.D. agents, or are general public figures. The only one who kinda does is Bruce Banner, but he wasn't even involved in Civil War. Introducing a registration act would do nothing because there are no identities to forcefully reveal.
2) Civil War involves waaaaaaay too many heroes that Marvel doesn't own the rights to; namely several mutants and of course, Spider-Man.
3) CW damn near turned Tony Stark into a villain; the repercussions of which Marvel is only just now able to slowly begin to reverse. I can't honestly see Downey's Stark going to quite the same extremes as comic Tony.
That, combined with the above reasons, means they couldn't possibly try to adapt Civil War without so drastically changing it to work around the aforementioned conditions that it wouldn't really be Civil War anymore. There's no point. There are SO many better, more adaptable storylines than Civil War. Notoriety doesn't equal quality.

I remember the forums back in the day and people would bay for Tony Stark's blood. I was one of them. But I love what Robert did with it and their spin.
Weak parts of the movies aside, his IM is to me an example of reconning a character for the better. Marvel films have been 50-50 on that.
And Marvel needs to get a grip before they ruin book Wolverine.
But to add to your thoughts...
I don't think there are enough heroes to reenact Civil War anyway-- that shit was long and had way too many well-established characters. Even if they got the licenses for the X-Men and Fantastic Four and Spiderman and got them all involved, it would take at least a movie of setup and then be extremely confusing and alienating to anyone who hadn't seen like the entire collection of everything else.
Also considering Tony makes the most bank out of all the Avengers' single movies I don't think they'll ever make him a "villain"-- even a villain by necessity as he so explained/retconned...
Also if they headed CW with Tony and Steve, that wouldn't work in MCU at least until the next Avengers movie because their relationship just isn't to that place yet and a miracle would need to be in place to properly convey that dynamic. Like what, are they just going to cut off the CW feature at PS Steve's dead! Nothing to see here?
So yeah, if this is actually a film thing and not a comic-reading thing I'll be really pissed/disappointed/extremely surprised and not in a good way.

With how little sense it made with the characters in 616 verse, it makes even less sense with them as they are in the MCU.
And thirdly they just can't because I'll fucking die if I see the "it wasn't worth it" speech played out by RDJ.
